Blood Pressure Development from Infancy to Adolescence

At birth, newborns have an average systolic pressure of about 70 mmHg and a diastolic pressure of around 50 mmHg, which is relatively low due to the small size of their heart and the low resistance in their blood vessels. As the baby grows, the heart and vascular system mature, leading to a gradual increase in blood pressure. Fat people have higher blood pressure. There is, however, an important confounding factor to be taken into account. There is a tendency to overestimate blood pressure in people with fat arms particularly if the blood pressure cuffs are too small. 

The fatter the arm, the greater the overestimation. However, after correction for arm circumstance, there still remains a positive relationship between body mass index (BMI) and both systolic and diastolic blood pressure. There is a suggestion that BMI exerts its effects mainly on diastolic blood pressure with little independent effect on the systolic pressure, which is mainly related to age. 

The mechanism by which obese people have a high blood pressure is uncertain. It is probable that obese people eat more sodium and less potassium and, therefore, may develop a rise in blood pressure due to dietary factors. 
It is probable also that high blood pressure is more closely correlated with central obesity than with BMI alone. Intravenous glucose tolerance tests have demonstrated that obese people are relatively resistant to insulin, and this concept is now the subject of a great deal of research. 

It has been postulated that insulin resistance leads to a rise in intracellular sodium concentration and to renal retention of salt and water. Thus insulin itself may play some role in the aetiology of essential hypertension, particularly in obese individuals. Since obesity is itself associated with high blood lipids levels, glucose intolerance and high blood pressure, fat people are more prone to coronary heart disease. 

It is possible, however, that after allowing for these factor. BMI alone may not itself be an independent cardiovascular risks factor. This is only a theoretical consideration as most obese people do have the other cardiovascular risk factors and thus have a high risk of death. When people lose weight, their blood pressures tend to fall at a rate of 1 mm Hg/kg reduction of body weight.

Shy-Drager syndrome

Stage III type of blood pressure

The severity of hypertension can be divided into four stages:
Stage I mild
Stage II moderate
Stage III severe
Stage IV very severe
In stage III it has diastolic pressure: 110 -119 mmHg while systolic: 190 -290 mmHg. In this Stage III type of blood pressure, the patient usually has serious health problems along with High Blood Pressure. Nocturnal hypertension

Nocturnal is the behavior characterized by being active during the night. Nocturnal blood pressure is the blood pressure measured during bedtime to arising or between 1 am and 6 am by ABPM or home blood pressure.
Nocturnal hypertension is diagnosed by the average of nocturnal BPs
>125/75 mmHg. The value was suggested by American Heart Association.

This threshold was lowered by the European Society of Hypertension updated practice guidelines that considered a nighttime blood pressure of greater than 120/70 mmHg to be abnormal.

Salt and sodium

The most familiar sodium compound is salt. Table salt is actually sodium chloride (40 percent sodium and 60 percent chloride). And sodium is not only found in salt shaker but also in foods and products that have been made with salt or one of its cousins, such as monosodium glutamate (MSG) and sodium bicarbonate (baking soda).

Sodium chloride is an ionic compound with a melting point of 800° C. Within a piece of salt, the ions are arranged in a structure called a crystal lattice, in which each sodium ion is surrounded by six chlorine ions and each chlorine ions is surrounded by six sodium ions. When salt is dissolved in water the ions are pulled apart and can move around.
Sodium is an essential mineral. Its main role is to help maintain the right balance of fluids in human body. It also helps transmit nerve impulses that influence contraction and relaxation of the muscles.

If the kidney can’t eliminate enough salt in urine, sodium starts to accumulate in blood. Sodium naturally attracts and holds water, which increases the total volume of the blood. This, in turn, makes the heart work harder to pump blood through the body, which increases the blood pressure.

Sodium intake in DASH diet

The DASH (Dietary Approaches to Stop Hypertension) diet promotes the consumption of healthy nutrients that have been scientifically shown to reduce blood pressure.

It primarily limits cholesterol and saturated fat while focusing in the increased consumption of foods packed with fiber, protein and micro-nutrients such as magnesium, calcium and potassium that can help in lowering blood pressure.  The DASH diet provided about 3000 mg of sodium/day, and amount only slightly less than the typical intake in the United States.

Most of the foods that are recommended have a very limited amount of sodium, and so should decreased intake of salt. When sodium content of this diet was lowered, there was an even greater reduction in blood pressure.

Lowering sodium also reduced blood pressure in the group consuming a typical American diet. With sodium, the DASH diet actually has two different types of recommendations. The regular DASH diet suggests limiting sodium intake to 2.300 mg daily, which it should be able to meet this goal without too much difficulty.

The alternative suggestion is a lower version of the diet reduces sodium consumption to 1,500 mgs a day and is even more effective for those sensitive to salt, including middle-aged to elderly adults, and who already have high blood pressure.

Magnesium lowers blood pressure

In general, a daily magnesium intake of 480 mgs or more lowered high blood pressure whole taking only the RDA requirements (360 mgs per day) did not.

Researchers have proposed that subclinical magnesium deficiency has developed in industrialized countries and this has paralleled the increased prevalence of hypertension.

Conversely, persons consuming vegetarian diets which are usually high in magnesium and fiber content, tend to have lower blood pressure than non-vegetarians, a finding raising the possibility that dietary magnesium is inversely related to blood pressure.

Magnesium helps to lower blood pressure by dilating blood vessels. Adequate magnesium intake allows the coronary arteries to relax and open wider to lower blood pressure.

Magnesium is a required cofactor for enzymes involved in a fatty acid metabolism and the synthesis of prostaglandins, which are known to include blood pressure.

Calcium and blood pressure

From epidemiological studies, there is only weak evidence of an association between dietary calcium intake and blood pressure, yet there are a number of biological mechanisms that suggest an important role for calcium in blood pressure regulation.

Result from epidemiological reports and animal studies suggest a threshold for calcium intake below which arterial pressure increases and a low calcium intake may amplify the effects of a high-NaCl diet on blood pressure.

In the human diets with less than 600 mg calcium/day are most clearly associated with hypertension. There was a study found that individuals consuming less than 300 mg/d calcium experienced 2-3 mmHg higher blood pressures compared to those consuming more than 88 mg/d calcium (Harlan and Harlan 1990). 

Because many with high blood pressure have a lower daily calcium intake than people with normal blood pressure, calcium rich foods, including nits and leafy green vegetables such as watercress and kale, should also supplement the diet.

Sodium and blood pressure

Sodium as salt – sodium chloride – was one of the first nutrients directly linked to hypertension. Salt contains 40 percent sodium and 60 percent chloride.

For years, high sodium intake was considered the primary factors responsible for high blood pressure. Then research pointed to salt that has a greater effect on blood pressure than either sodium or chloride alone or in combination with other ion.

Researchers generally believe that a kidney in ability to excrete salt is responsible for salt-induced high blood pressure. To compensate for this inability, the body increases blood pressure so the kidney can filter more salt, which then enters the urine.

This increased blood pressure helps eliminate more salt, but it also puts a strain on the body’s arteries and sets the downward spiral of blood pressure damage in motion - a vicious cycle.

The average American gets more than 3600 milligrams of sodium daily, the equivalent of about 9 grams of table salt. Blood pressure increases in response to excess in salt intake – most notably for those with hypertension.

Those with blood pressure should consume less than 2 grams of dietary sodium per day. This can lower blood pressure 2 to 8 mmHg.

Alcohol actually has two effects on blood pressure. One of these effects was initially hidden from earlier researchers, because they often performed blood pressure measurements 12 to 15 hour after the original alcohol intake.

It turns out that this point coincides with the maximal effect that alcohol has increasing blood pressure.

However, when researches attached a monitor that took study participants’ blood pressures multiple times each hour and measured their blood pressure responses during the time immediately following alcohol ingestion, and then continued on to take their blood pressure measurements 12 to 15 hours or more after alcohol intake, a different picture emerged.

Shortly after alcohol ingestion, there is actually a decline in blood pressure. After a few hours, blood pressure returns to its baseline values. As some of the metabolites (the breakdown products of alcohol) the n enter into the circulation, there is actually an increase in blood pressure that is evident about 12 to 15 hours after the original alcohol ingestion.

Overall, the net effect of alcohol is an increase in blood pressure of about two points, or 2 mm Hg.

If you look at the blood pressure 12 to 15 hours after ingestion, this effect appears to be more like 6 mm Hg.

Because a 4 mm Hg decrease in blood pressure occurs after ingestion, however, the net increase from alcohol is actually about 2 mm Hg.

Celery and blood pressure

Celery is an excellent source of antioxidant nutrients such as vitamin C, beta-carotene and manganese. Celery contains calcium blockers and other plant chemicals that help prevent and treat arrhythmias and other compounds that help lower blood pressure and cholesterol.

Celery is high in apigenin, a chemical that expands the blood vessels and may help prevent high blood pressure. Celery is a traditional Asian folk remedy for high blood pressure.

In 1992, Minh Le, the father of a University of Chicago medical student was diagnosed with high blood pressure. His doctor prescribed standard antihypertensive medication.

Researchers at the University of Chicago Medical Center have found that very small amounts of a compound in celery, 3-n-butyl phthalide, can lower blood pressure in animals by 12-14 percent and cholesterol by about 7 percent.

It’s known in scientific circles that phthalide relaxes the muscles and arteries that regulate blood pressure.

Isolated diastolic hypertension

Systolic blood pressure rises in a linear fashion with age, whereas diastolic pressure increases until the age of 50 then levels off and even begins to fall.

When hypertension is diagnosed in middle age, the most common blood pressure pattern is elevated diastolic pressure with systolic pressure being either normal or elevated (combined systolic/diastolic hypertension).

Isolated diastolic hypertension is more common in men and often associated with middle-age weight gain and the metabolic syndrome. It is also linked to a high heart rate and cardiac output suggesting high sympathetic nerve activity. It may account for up to 14% of hypertension in the elderly.

Isolated diastolic hypertension is thought to evolve primarily from normal or high-normal blood pressure. Follow up suggest that individuals with isolated diastolic hypertension later develop mixed systolic diastolic hypertension.

Isolated diastolic hypertension does not have any significant impact on the clinical sequelae of hypertension. Beyond the age of 60, a decrease in diastolic blood pressure and a wide pulse pressure have correlated with an increased cardiovascular risk.
